Glomerulonephritis in Children

Inflammation of the kidneys and upper tract may be acute or chronic. Acute or chronic inflammatory disease resulting from infection may involve the kidneys and upper Urinary tract (pyelonephritis) or the bladder and lower tract (cystitis).

Whether one believes it or not, the disease in question is caused as a result of a sore throat. It may be tonsillitis/pharyngitis, or both. However, not all cases of sore throat cause this serious disease of the kidneys. It only occurs when the sore throat takes place due to the invasion of a specific bacteria called group A beta-haemolytc streptococcus.

A urine examination shows marked red blood cells (RBC), there may be a small amount of albumin in the urine and the blood urea will be slightly raised. There will be no pus cells in the urine, and the urine culture will be sterile as there is no direct invasion of bacteria, and hence no infection/pus formation.

Nephritic Syndrome, also known as Rapidly Progressive Glomerulonephritis, is seen more in adults. This is characterized by reduced urine output, presence of blood and protein in the urine, and swelling in the body. This disease also results from damage to the filtering mechanism in the kidneys. Mostly the cause is unknown, but it is also believed to result from a malfunction of the immune system of the body.

Pharyngitis (an inflamed or sore throat) is an infection that has a fairly typical set of symptoms associated with it although it can be caused by many different organisms. Children with sore throats almost always have: painful swallowing; neck soreness in the glands right under the jaw; decreased eating, especially of salty or "sharp" food; and decreased drinking, especially of acidic drinks like juice.

Symptoms are proteinuria, hematuria, decreased urine output (oliguria), swelling, increased blood pressure. General symptoms include: fatigue, fever, nausea, vomiting, headache, lack of appetite. Typical symptoms are swelling around the eyes and face. Urine is colored brown or bloody. In the urinary sediment are erythrocytes and different cylinders.

A healthy person usually excretes urine of clear or slightly yellow color. At times, one might notice a discoloration in his or her urine. Sometimes, the discoloration may be due to the food that one consumes. Eating beets, for example, can cause urine to be a reddish color. Eating excess amounts of asparagus, on the other hand, can cause it to turn an unusual shade of green.

You usually get strep throat from another person. Strep can be passed by touching a surface that has strep on it or from an airborne passage of strep from one person to another person. Children playing side by side can easily pass strep from one child to another child.

Children under the age of three rarely develop streptococcus infections and most of the cases do not originate in the throat. The most exposed patients to sore throats triggered by the strep bacteria are children between 5 and 15 years of age. Most often, streptococcus infections occur during the months when children are in close contact with each other.

It is necessary to see clearly if there is a flu or a strep throat. So, there must be performed some tests: rapid strep test, and a rapid influenza test. If these tests turn negative, and clinical signs warrant it, there may still be necessary a blood test that evaluates the patient's white blood cells, and blood cultures may be drawn, to exclude the risk of sepsis.
